Ongoing aftercare
We stay connected with regular check-ins, clear communication, and access to ongoing care whenever you need it.
A telehealth consultation is simply a medical appointment that happens online, usually through a secure video call on your computer, tablet, or phone. It's super convenient because you can chat with your doctor or nurse practitioner from the comfort of your own home! We'll send you a secure link before your appointment, and you just click it to join. Easy peasy!
You'll need a reliable internet connection, a device with a camera and microphone (like a smartphone, tablet, or computer), and a quiet, private space where you feel comfortable talking openly. It's also a good idea to have any relevant medical records or a list of your current medications handy.
Our doctors focus on a range of conditions where a personalised approach to care can make a real difference. This often includes things like chronic pain, stress, sleep difficulties, and other ongoing health concerns. Your doctor will assess your unique situation to see how we can best support you.
Yes, minors can have consultations with our clinicians. However, for all appointments involving a patient under 18, a parent or legal guardian must be present during the entire consultation. The parent/guardian must also book in the appointment, complete the patient intake form, and provide consent for their care. We may also require a letter of support from the minor’s regular GP and paediatrician or relevant specialist. This ensures we meet all legal and safety requirements for our younger patients.
Following your consultation, your clinician will discuss your personalised healthcare plan and recommend next steps. If appropriate, consultation information and ongoing care details will be available through our Azure Patient Portal. Our team will continue to support you with any follow-up requirements and provide access to relevant information throughout your care journey.
Our consultation fees vary depending on the type and length of the appointment. We'll provide clear pricing information upfront on our website. Please note that Medicare rebates are generally not available for telehealth consultations. Reach out to us, so that we can help you understand your options.
Further information about our current research trials and your eligibility to participate will be available in the Azure Patient Portal following your initial consultation.
Your privacy is our top priority! We use a secure, encrypted telehealth platform and follow strict Australian privacy laws (like the Privacy Act and Health Records Act) to protect all your personal and health information. We only share your data with your consent or when legally required.
Our caring support team is here for you! If you have any questions about your treatment plan, your next steps, or need technical assistance with the Azure Patient Portal, you can reach out to us via 1300 020 224 or support@azurehealth.com.au. We're here to support your journey.
If you have opted out of My Health Record or restricted your My Health Record and don’t allow our clinicians access, then you’ll need to provide a GP Referral with a Health Summary. A Health Summary includes info on your medical history, adverse events and allergies, and current medications. This is so our clinicians have a baseline to work from and can safely support you along your journey. You can ask your GP to send your referral to us.
If you haven’t restricted access to your My Health Record, then you don’t need to do anything - our clinicians will simply check your My Health Record as part of your initial consultation. If you have restricted access to your My Health Record, then you’ll need to provide us with the record access code (RAC) during your patient onboarding. You can find more information on granting organisations access to your My Health Record on the Australian Digital Health Agency website.
We're always here to help! Please feel free to contact us directly at 1300 020 224 or support@azurehealth.com.au
